Guidelines for Requesting a Birth or Death Record and a Genealogy Search:

• Birth or Death must have occurred in Boone County and after 1882
• Mail completed application to the following address:
  116 W. Washington St.  Suite B202, Lebanon, IN 46052
• Provide a clear copy of a valid driver’s license or State issued photo ID
• NO out of state checks accepted. Please send a money order or certified check.
• There will be an additional fee for the use of a credit card.
• We cannot offer genealogy services on a walk-in basis.  Please allow 6-8 weeks for processing.
• If a genealogy search for a birth or death occurred between 1882-1920, we may not be able to supply complete information.
• Move fee for records down
• Move form for genealogy services under vital records services and resources
• Move the rest of the information as it is being removed to bullets under guidelines but keep the last line to contact and replace the email address with: mmcneeley@co.boone.in.us

Fees for Records

• Certified Birth Certificate: $15.00 each
• Certified Death Certificate: $15.00 each
• Amendments for Birth certificates: $10.00 each
• Home Birth Registration: $35.00 each
• Paternity Affidavit: $25.00 each

Boone County Health Department does not accept out-of-state checks

We are not able to offer genealogy services on a walk-in basis.  Please allow 6-8 weeks for processing.

If a genealogy search for a birth or death occurred between: 1882-1920, we may not be able to supply complete information.

In addition to cash, check or money order, we accept Debit, Visa, MasterCard and American Express for payment. Please be advised that there is an additional fee of $1 per transaction + 2% of total purchase when paying via card.
