Several forms require the associated parcel number.  This number can be obtained from the GIS system on the web-site, by calling the County GIS department (765-483-4406) or by calling the County Auditor’s office (765-482-2940).

Septic Permit

Septic, Well & Pump Permit (New)

Use this form to initiate a request for a permit for a new septic system, a new water well and / or a new water system pump. This form can be filled out by the owner, builder, or installer.

Please fill out at a minimum the first two sections of the application and submit it. The Septic Inspector will be in touch to complete the balance of the form.

Septic Repair Permit

Use this form to initiate a request for a repair permit for your existing septic system.

Food Permit

Application for Plan Review

Fill out when building a New or Remodeling of a Restaurant or Eatery. This is required to allow the County Health officials to confirm the design of the facility.
