Reporting a death:

If this is an emergency call 911.

If you are a hospice provider or health care provider and need to speak to an investigator contact Boone County Dispatch at 911 or the non-emergent line at (765) 482-1412.

For Pick up of Remains:

Decedents can be picked up from our facility during normal business hours Mon-Fri 8 am – 4 pm by appointment.

Contact our office at (765)483-3206 to schedule.

Pick up is conducted at our office, 1905 Indianapolis Ave, Building C, Lebanon, IN 46052.

Enter the main property entrance. Follow the signs to the Coroners Office. Transfer of remains occurs inside door 27 (garage door). Call the office upon arrival.
