Juvenile travel permit When a juvenile is leaving the state, parents must complete the travel permit at least one week prior to traveling.  The permit must be submitted to the probation department and must be approved by the Court.  Juveniles are not allowed to vacation with friends without parental supervision. 
Petition to Expunge Juvenile Records

When a juvenile has successfully completed their term of probation, they may petition the Court to expunge their juvenile delinquency records.   The petitioner must be at least 18 years old and have no further involvement in the legal system.

 

The Court reviews the file and, if expungement is accepted, all records of law enforcement, prosecutor, and juvenile probation will be shredded.

Juvenile Write-In Forms If permission is given by the Court for Write-In Reporting (long distance; college), the juvenile will complete the form and either email or fax it to probation.  Complete the form and note any changes such as email address, address, or phone number.  Write-Ins should be completed by the 5th of each month. 
Fourth Amendment Waiver If a fourth amendment waiver is ordered by the Court, the juvenile will sign a form waiving the U.S. Constitution 4th Amendment Freedom from unreasonable search and seizure.  This allows law enforcement, probation, or Community Corrections to search for any alcohol, illegal drugs, or paraphernalia without a warrant or reasonable suspicion.  The juvenile agrees to report to the probation department for a drug screen when requested.   The 4th Amendment Waiver is only in effect for the length of probation or involvement in the legal system.
Wardship Information Sheet This form is to provide Probation and the Court with the name of the parent(s)/guardian(s) of the child and their contact information to the best of your knowledge.  This information is required to be supplied when completing state-required forms.
E-NOTICE Form Boone County Courts are now notifying parent(s)/guardian(s) of upcoming hearings through email if it is available.  If you do not have an email address, a hard copy of Court records will be mailed to the parent(s)/guardian(s) of the juvenile.
Dispositional Alternatives This form must be provided to the juvenile and parent prior to the Preliminary Inquiry being conducted.  This is to inform the family of all possible dispositional alternatives once a juvenile becomes involved in the legal system.
Preliminary Inquiry Information

A Preliminary Inquiry will be completed by a juvenile probation officer when an allegation of a criminal offense has been reported.  The purpose of this report is to gather information about the juvenile and to identify what services the child and family need.   This information is shared with the Court and Prosecutor to help in determining the best services needed.

 

Prior to the Preliminary Inquiry, parent(s)/guardian(s) and child need to read and sign the Wardship Information Sheet, Advisement of Rights, and the Dispositional Alternatives.

Community Service Record Sheet

This form is to be used for anyone who has been ordered to complete community service.  The supervisor of the organization where you complete your community service will sign the days and hours works.  They also provide a phone number for verification.  All community service must be completed at a non-profit organization. 

 

GPS Schedule for Community Corrections This form is to be completed weekly and turned into Community Corrections each Monday.  Juveniles on GPS are permitted to attend school, counseling, and work if they are employed.  They are required to report to probation weekly and keep their probation officer informed of any changes. Home visits will also be conducted by staff of Community Corrections as they see fit.
Incorrigible Packet This packet is completed by parent(s)/guardian(s).  There is an attached Probable Cause form that must be completed.  An Incorrigible child is a child that repeatedly refuses to follow reasonable requests made by parent(s)/guardian(s).  Please list in detail the problems you are experiencing in the home and relationships of family members.  List prior and current counseling, any diagnoses, and medication the juvenile is/has taken.  A Preliminary Inquiry will be completed when the completed packet is returned to probation.  The Prosecutor will review the packet along with the Preliminary Inquiry and make a decision if a Delinquent Petition is filed.  Most Incorrigibles are handled as an Informal Adjustment (IA).  If placed on an IA, the juvenile would be placed on informal probation for a period up to six months.  They will be requested to waive their 4th Amendment rights and take any educational programs probation feels would benefit the juvenile and family.  Parents will be required to participate in any counseling that is referred to the family.  Please remember that if you request probation, you will be required to follow through with recommendations even if things are better at home.
Support Group Verification This form is for verification of attendance to AA or NA meetings.  Each meeting attended must be signed by the chairperson of the meeting.  The form is to be provided to your probation officer at each meeting.
T4C Program Agreement When a child is ordered into the program of Thinking for a Change, the juvenile and parent(s)/guardian(s) must complete the program agreement and return it to probation.  Fees for the program must be paid prior to the first class.  
Advisement of Rights Prior to the Preliminary Inquiry, the child and their parent(s)/guardian(s) are to read and sign the Advisement of Rights form. You are not waiving any rights but stating that you and the child have been provided with the juvenile’s rights.  The juvenile has the right to have counsel present during the Preliminary Inquiry if they choose.  They also have the right not to answer any question they do not feel comfortable answering.
