Guidelines for Requesting a Birth or Death Record and a Genealogy Search:
- Birth or Death must be recorded after 1882
- Mail a completed application to the following address: 116 W Washington St Suite B202, Lebanon, IN 46052
- Provide a clear copy of a valid photo ID
In regards to a genealogy search: If the birth or death occurred between 1882-1920, we may not be able to supply completed information.
Fees for Records:
- GENEALOGY Services: Includes Uncertified Record
Name Search (Birth or Death): $15 each
Please be aware that we are not able to offer this service on a walk-in basis due to staff availabiltiy. Please allow 6-8 weeks for processing.
- Certified Birth Certificates: $10.00 each
- Certified Death Certificate: $10.00 each
- Amendments for Birth certificates: $10.00 each
- Home Birth Registration: $25.00 (includes Birth Certificate)
- Paternity Affidavit: $20.00 (Includes Birth Certificate)
- We accept Visa, Mastercard and American Express along with cash, check or money order for payment. Please be advised that there is an additional fee of $1 per transaction + 2% of total purchase when paying via card.
